Please print out and submit one per camper
The following forms are required:
- Parent Questionnaire
- Camper Questionnaire
- Medical Form (please email to receive a copy of the medical form. You cannot get this form on the website.)
The following forms are optional, please print and submit if applicable to your camper(s):
- Bar/Bat Mitzvah Tutoring request form
- Transportation forms:
- NY/NJ/CT Bus sign-up
- Manchester airport information (for those flying to camp)
CampMeds
We will send out an email in the spring explaining how to register with CampMeds. If you are already familiar with the process, you may register your camper if your camper will be on medication over the summer.
